Raffle Mechanics:
1. Promo is valid from November 2 – December 22, 2011.
2. Promo can be availed in the following Belo surgical clinics: Tomas Morato, Greenhills, Medical Plaza, Alabang.
3. To be eligible to join the raffle promo, patients must avail of any of the following procedures: Rhinoplasty-Silicon, Ulthera, Blepharoplasty and BodyTite get 1 raffle entry for every procedure. For Smart Lipo, patients will get 1 raffle entry for every Php 50,000. For Rhinoplasty – Goretex procedure, patients will get 2 raffle entries.
4. Patients shall be given a raffle entry by the Belo Finance representative upon full payment of the procedure at the Cashier. The patient shall fill up the raffle coupon with Name, Address, Tel #, Email and affix signature and drop in drop boxes located at Belo surgical clinics. (Tomas Morato, Greenhills, Medical Plaza, Alabang)
5. Patients to drop off raffle coupon in the drop boxes located in the selected branches (Tomas Morato, Greenhills, Medical Plaza, Alabang).
6. Patients will get a chance to be be one of the 5 winners of Rustan’s Gift Certificates, one of 3 winners of iPad2 or the lucky winner of a trip to Misibis Bay for 2. Prizes are not convertible to cash but transferable.
7. Deadline of submission of entries is on December 22, 2011.
8. Draw date will be held on December 23, 2011 at the Belo Marketing Office with office address: Suite 1404 Medical Plaza Makati, Amorsolo Street, cor. Dela Rosa Sts., Legaspi Village, Makati City.
9. Employees of Belo Medical Group and participating outlets including their relatives up to second degree of consanguinity or affinity are not eligible to join.
10. This promo may not be used in conjunction with other Belo promos, programs, tie-ups and discounts.
11. A person can only win once. Should the person’s name be drawn more than once, that person will get the higher prize and the lower prize will be available for awarding to another person.
12. Belo Medical Group shall pay 20% tax for prizes exceeding Php 10,000
13. Winners will be notified through registered mail.
14. Winners must claim their prizes within 60 days from written notification, otherwise it will be forfeited in favor of Belo upon approval from DTI.
15. To claim the prize, winners must present a valid ID, registered notification at Suite 1404 Medical Plaza Makati, Amorsolo Street, cor. Dela Rosa Sts., Legaspi Village, Makati City.
